• One entry per person per Quiz. Print your answers and name clearly. Acronyms are not preferred. Spell out your employer, firm, etc. We do not need titles.
• Submit your entry form by mailing or faxing it to the Long Beach Business Journal. Entries may be faxed to 562/988-1239 or mailed to BIZ QUIZ, Long Beach Business Journal, 2599 E. 28th St., #212, Signal Hill, CA 90755. You may send entries from several people in a single envelope to save postage. Entry forms may be dropped off at this address as well.
• Entry forms may be copied for others to fill out and submit.
• You may submit entries for other individuals as long as they are aware of it. If they win, they must call to claim their prize, not the person who submitted the entry. IDs may be requested.
• Winners must call the Business Journal by the date indicated to claim their prize. Those who do not, forfeit their prize.
